    It has a religious purpose, with that cross and Our Lady of the Miraculous Medal on the lid. I can't discern the other figures, but they are probably religious as well.

    Judging by its size, it may be a pxy from a travelling communion set.

    It had no bottom round piece, just the top one.

    Maybe the lid doesn't really go with this and it is a napkin ring.

    The overall image is of the Four-Way Medals. I'd guess it was a lid for a rosary box. It just happened to fit the napkin ring.

    Maybe the rosary box died and they needed something that fit. It's bang on size for a traveling Communion set, but a 4way medal on hosts would be weird. I'd think hosts would have a crucifix. (but i'm a Gentile; what would I know?)
