Antique wrench?????

purchased this with several other tools and I have no idea what it is.
The pieces are numbered 1 - 4
It ratchets both ways when you turn the wing nut.

Looks like a very old ratcheting socket wrench.

I did try some image searches but didn't find anything. Perhaps try an antique car forum? Possibly a handle for a jack? Very cool though...

It's for sure for heavy duty turning/twisting.
As for swing bridge or a train exchange or something. Industrial purpose perhaps? Heavy door? Heavy floor shutter?
