Here is a better example of a string game by a major carver, Johnny Inupuk, at the height of his powers. string.jpg

Buzz toy consisted on a flat wooden disk with two holes pierced near the center and there were two small pieces of wood that were attached to a doubled cord that goes through the two holes. The cild holded a handle in each hand, the disk was wound by whirling the string until it is twisted tightly. Then the string is pulled and released alternately. The Indian children would use their spinning disk to touch another child’s disk and try to stop it from spinning.

Komokwa could very well be correct but in either case it is a game rather than intestine or hunting rope, etc. etc. Sadly, the carver of the OP's piece was not at the top of his/her game.

We use to make a similar toy with a large 2 hole button. You would wind it up with two hands by turning the button rotating it foward round and round and the string would get shorter and shorter. Then pull outward and the button would spin backward and forward and your hands would be pull in and out. And it would make a beautiful sound. That was in WVA back in the 50's. We did not buy toy we made our toys. One of my favorite toys was a metal bike rim from a bike and a metal clothes hanger running around with the chothes hanger pushing the rim. Way before Xbox.
