Austro-Hungary Diana hallmark for 800 silver pre ww1, 11 cm hight ,with 8 wide on the bottom side. About 40 grams.
Not sure , is it an ex-voto plaque or something else ?

It looks like an ex voto, but an ex voto wouldn't need so many holes to attach it to something.
Maybe it was attached to a religious garment? Possibly a cloak worn by a Madonna?

It does look as if it were designed to be securely stitched to something. Crude examples but... The Madonna's gown often displays a pierced heart and a pierced heart is often used to secure her cloak.

A Madonna with seven daggers through the heart, like the one on the left, is Our Lady of (Seven) Sorrows.

When Mary and Joseph took Jesus to be dedicated at the Temple, Simon told her that a (probably figurative) sword would pierce her heart. This is definitely related to Mary, even before you add in the roses. This Protestant isn't too up on her Catholic symbology, but yup.. Mary for sure.

There is a religious symbolism to it, but its actually a mix of 2 symbols, or maybe even 3.
The first one is the flaming sacred heart of Jesus. The leaves on top of the heart are actually flames.
Then you have the sword and roses that are a symbol of Mary.
I say 3 symbols because you also have the flowers that are on top. Lily ? Flowers i mean, not sure how they tie to it. I think that type of flower is a symbol of death.

    I am an overthinker )
Could it be a masonic object ?

They do sacred heart of Mary too, as a symbol. There's nothing Masonic in sight.
