It appears to be a pendant with Hebrew writing & Jewish significance.

It's fairly recent, if not brand new. Nothing old about it except the Hebrew on the inscription. The design is...odd, to put it mildly. The inscription probably tells us what it is and what it's used for, but we can't see the whole thing.

Does the cord that runs over the top move, can it be pulled side to side?

My feeling is that it is for counting, or otherwise keeping track of prayers, but I do not know enough about Jewish prayer traditions to say how or even whether this makes any sense at all. If an elderly man in Israel did not recognize it...

It could be a VERY modern take on a tallis clip. The Hebrew on the inside is the kind used in religious writing - no vowel marks. I can't see the whole inscription and my Hebrew isn't good enough to figure it out without more of a sample. I'm guessing it's the Shema - Shema Israel Adonai elohenu, Adoni echad etc...
