Lithuania, which used to be Russian, hence the Cyrillic script.
The knight on horseback with the Lorraine cross on his shield is grand-duke Mindaugas, a Medieval Lithuanian ruler. He is also featured on Lithuanian assay marks, that's how I recognized him.;)

@Figtree3 or @Aznathalie can help you with the text.

I wonder if the 'friends of winter' could have something to do with several of the northern parts of Tsarist Russia? So Lithuania and two others?

Under the princely crown on an ermine mantle in an oval shield is the coat of arms of Prince Golitsyn.

so anyone got any thoughts as to whom this medallion may have been given and why? I bought it at the weekend in an auction, it came with a watch which I bought and didn't realise this was included in the sale. Had a note inside a leather pouch saying it was of no significant value, which I am guess is probably accurate?

Figtree3 What would you do if you weren't afraid?

Around the edge on the back it says something like "St. Petersburg Society of Sportsmen on Horse Winter Race." The center refers to the person who is described in the article @Aznathalie just linked. The coin/medal/token says "Vice-President His Highness General-Adjutant Prince Vladimir Dmitrievich Golitsyn 1872."

Members of the House of Golitsyn were nobility in Russia. They were said to be descended from a prince of Lithuania. The front of this coin has the Golitsyn coat of arms.

The words on the front of the coin are in Latin.

Whoops, I didn't see her earlier post!
