    I don't think she is enamel, I think she is German porcelain, just like the Greek girl I mentioned.
    So painted, or more likely printed on porcelain, and glazed.

    ...with a clear glaze over top. Or clear enamel. The clear bit is scratched while the colors underneath are untouched.
