I'd guess that was carried on a belt and/or on a button hidden inside a coat. Odd the brush or I'd guess Laudanum or even Absinthe?
Perhaps it had/has to do with rotating specific grinds to find the best one and position for each eye?

I would think that technique was partly to make the barrel stronger. Maybe the metal was not very stable and strong.
