Skookum dolls aren't really my thing, but this one looks consistent with many I've seen, and I wouldn't be concerned about the toilet paper tube....

It is from the Island of Lombok, in Indonesia. A pretty common import store item as you mention, probably worth more than $4, but not a lot more.
