If the 2 are near mirror images of each other, I would guess faux. Don't think they are quite so symmetrical in nature.

My thought is that it is like a bar brooch in disguise. You could certainly wear it to keep a collar closed.
Seems like a risky way to attach them. My guess is that there is a pin inserted into the pearl, but someone reinforced with glue.
