I have never viewed anything like it in modern jewelry. Just a wild guess, 2 stones the same size and drilled in the same location could be pegged...
What is inside of the hole,can you scrape out the material with a needle? It doesn't look like the "flat" inside the hole was drilled.
With the drilled hole and the crack and chip on the back,I would guess there are 2 stones in your setting. Backside onyx and front side sardonyx.
