I don't see tea stains,it has been scrubbed violently and worn off the black glaze into the "brown" lower layer.

In the bottom photo the far right diamond looks different than the others in the photo. The "table" on diamonds should be 53-58% of the stone...

Looks like 3 domed cold connection beaded pins.
I don't see anything in the photos to suggest the stones weren't glued in to start with.Unless there is something in the way the stones were cut...
The designs on the whole box have been hand engraved or did you mean no owners initials?
