Any idea if this is shell or something else?

Could be, but there is no way of telling with single beads like that.

I have nothing like your scientific knowledge of sea shells, other than that they can develop 'Byne's Disease', but since I have been collecting cameos for some years, have lots of experience with shell cut for use in jewellery & I'm not seeing shell here, or not in the blue bead. Still not clear on white beads. I also have a handful of ivory pieces& blue beads look more like that.

Front of a helmet shell cameo:

    Back of same piece:

But these were taken with a USB microscope; you don't see growth lines without magnification.

Helmet shell when cut the other direction has easily visible layering:

I am sensing a bit of "blind men and elephant" allegory here... I think we all have a bit of everything, but not all of it. Yet. That means there's more factual proof out there that would tie it together. upload_2019-5-30_16-49-38.png

... but my enthusiasm is waning.

Puka is Hawaiian for "hole",it was naturally occurring from erosion to cone shells with the surf and beach sand. The OP's necklace does not have natural Puka shells.

I'm getting a glass trade bead vibe from the cores of the blue ones:
