Going through more of my Dads photos and I came across this one from the 20's
Never seen it before and it was never discussed, the man is from my Dads maternal side who came from Sweden in the early 1800's

It looks more like a wounding or injury rather than surgery.

Known in Scotland as a Glasgow Smile or London as a Chelsea Grin.

If a wound, it's very symmetrical.

chelsea smile was the first thing that came to my mind as well, the thought of it being done to a woman is beyond horrendous

    https://allthatsinteresting.com/glasgow-smile

In 20th-century Scotland, roving gangsters punished each other by carving the sides of a victim's mouth into a demented grin known as the "Glasgow smile." But this bloody practice didn't end there.

Inflicted by cutting from one or both corners of the victim’s mouth, sometimes all the way to the ears, the so-called Glasgow smile originated in a dark period in the Scottish city of the same name. The victim’s screams of pain only served to tear the cuts open further, resulting in a terrifying scar that marked the wearer for life

Ditto. The same edge pattern that creates the 'grin' continues in a straight line along the upper lip. A cut on the lip would result in a much more uneven scar, different from a scar on the cheek.
Voilettes, or short face veils, could be any length from just above the eyes to below the chin.

Yours truly with a voilette, in my modelling days (ancient history):
